Insurance

Our office accepts a variety of insurance plans to accommodate our patients needs. We currently accept the following plans.

• Florida Blue Cross Blue Shield / BCBS
• Avmed State of Florida
• Medicare

We can also file other insurance plans on your behalf if your plan is not listed. Please check with your carrier for out of network benefits and policies. We have filed Blue Cross Blue Shield Federal and Avmed non state of Florida plans successfully, as well as some United Healthcare on a case by case basis.

Our staff is also happy to provide any patient with a SuperBill(Universal health claim form) to assist them with their private claims submission.
